CIM Forms

FAFSA - online at www.fafsa.ed.gov Required each year to apply for or to renew aid for U.S. citizens and eligible non-citizens, excluding Merit Only applicants/students. For 2008-2009 applicants, the due date is February 15, 2008. The returning student due date for renewal FAFSA is March 1, 2008.

08-09 International Student Declaration and Verification Form This form is required of all international students applying for admission for the 08-09 academic year.

US Enrollment Verification Form Required of all international applicants who are currently or have attended another U.S. school (high school or college).

Transfer Clearance Form is required of any applicant who will NOT have completed their current degree at their prior school before enrolling at CIM. This is also required if you have previously deposited and signed an Intent to Enroll Form with another school (this may happen in you are wait-listed at CIM, and accepted another school’s offer in the meantime).

2008-2009 Intent to Re- Enroll and Renewal Aid Form Required of every returning student by February 1, 2008.

2007 Work Study Contract - required of all Work Study awardees - each year. Only complete if asked to do so by the CIM Financial Aid Office.

2008-2009 Work Study Contract - same as the above, but for the 2008-2009 year.

Payroll New Hire / Tax Forms (required of all new Work Study, Service Award and Fellowship recipients)

Special Circumstances Appeal Form Use this form to appeal for a re-evaluation of your financial aid package. You must have specific, documented financial changes either to student or parent finances or family situation that is new in 2006 from the data submitted on your current FAFSA.

Outside Scholarship Information Form - use separate form for each outside award – outside awards are always credited one half to each semester, like all other financial aid, irregardless of the disbursement frequency from the source of the award.

Perkins Loan Master Promissory Note - colFinAidCimPerkinsMPN.doc – Only submit if you have been awarded a Federal Perkins Loan from CIM and did not have a Perkins Loan at CIM last year.

Perkins Loan Entrance Counseling - colFinAidCimPerkinsLoanEntrance.doc – Reguired of all first time Perkins borrowers at CIM.

Direct Loan Master Promissory Note dlenote.ed.gov/empn/mpnwizard.jsp - required of all first time Direct Loan borrowers. Must be done with the student PIN (not parent). This in an interactive site, not a form per se. * CIM is notified electronically when this has been completed

Direct Loan Entrance and Exit Counseling www.dlssonline.com– required of all first time Direct Loan borrowers or those with Direct Loans in their final semester at CIM. PLUS Loan Master Promissory Note dlenote.ed.gov/empn/mpnwizard.jsp (Eligible for US citizen or Green Card holder undergraduate students only) - required of all first time Direct Loan borrowers. Must be done with the student PIN (not parent). This in an interactive site, not a form per se. * CIM is notified electronically when this has been completed

CIM Loan Master Promissory Note - only submit if you have been awarded a CIM Institutional Loan and asked to complete by the CIM Financial Aid Office (only eligible to international students)

PLUS Loan Application 2007-2008 (for parent of undergraduate student OR graduate student as borrower. ).

Verification Worksheet (only complete if asked to do so by CIM Financial Aid Office)

• Dependent Students (generally this is for all undergraduate students, if not sure, check with Financial Aid Office)
• Independent Students (graduate students)

All students required to complete the Verification process must also submit a completed 08-09 FAFSA Worksheet ABC

Private/Alternative Loan Rate and Terms Comparison (PDF) Although you may use any lender of your choice, this document compares information from several lenders often used by CIM students.

FERPA Release Form - To permit CIM to speak to a parent, spouse, other person about student-specific information (such as tuition balance due, financial aid, grades, etc.) CIM must have the student consent in writing. FERPA regulations only apply to confirmed students (law does not apply to applicants, until they have confirmed enrollment). All admitted students are sent this form, but need only return to CIM if/when confirming intent to enroll.
