Financial Aid: Links & Forms

CIM Forms
- CIM Financial Aid Information Guide - document that explains in detail the aid offered by CIM, policies, regulations and other general information.
- Financial Aid Timeline for Applicants and Timeline for Returning Students - documents listing important dates for Financial Aid processing.
- FAFSA online (Free Application for Federal Student Aid) - Required each year to apply for or to renew aid for U.S. citizens and eligible non-citizens. For 2012-13 applicants, the due date is February 15, 2012. The returning student due date for the renewal FAFSA is March 1, 2012. CIM's School Code is 003031. For applicants this is required even if seeking only scholarship aid.
- Use this form to assist with your gathering of data needed to complete your 2012-13 FAFSA online at www.fafsa.gov. Do NOT send this form to CIM. This is simply a worksheet for your records.
- CSS Profile - This is required of all undergraduate U.S. citizens and permanent residents as well as all international applicants (both undergraduate and graduate) who seek any financial aid, even if only scholarship. CIM's PROFILE code is 1124. For 2012-13 applicants, the due date is February 15, 2012.
- Graduate Student Additional Information Form - This voluntary form is intended to provide graduate applicants (U.S. citizens and Permanent Residents) an opportunity to document any additional factors for financial need consideration that would not otherwise be known to CIM, as they are not part of the FAFSA data requests. For undergraduate and international applicants, all of this may be disclosed in the explanations/special circumstances section on your CSS PROFILE.
- I-20 Certification of Finances - Required of all international students to confirm enrollment. This should not be submitted until after a student has been admitted and will be confirming enrollment at CIM.
- Transfer Clearance Form - Required of any applicant who will NOT have completed their current degree at their prior school before enrolling at CIM. Also required if you have previously deposited and signed an Intent to Enroll Form with another school, such as if you are wait-listed at CIM, and accepted another school's offer.
- Intent to Re-Enroll and Renewal Aid Form - Required of every returning student by February 1 each year.
- Work Study Contract - Required if awarded Work Study as part of your aid package.
- Work Study Job Descriptions / Pay Rates - Descriptions and pay rates for work study jobs (new version attached effective 1/1/11).
- Outside Scholarship Information Form - use separate form for each outside award - outside awards are always credited one half to each semester, like all other financial aid, regardless of the disbursement frequency from the source of the award.
- studentloans.gov - This site is a one-stop shop for all Direct Loan processes, such as completing Master Promissory Notes for Stafford and PLUS Loans, completing Loan Entrance Counseling for first time borrowers and where PLUS Loan applications are done. The site is also a resource for current and prior students access to their loan borrowing history.
- 11-12 Federal Loan Cancel / Reduce Form – Please use this form when requesting a cancellation or reduction of any previously accepted federal loan (Stafford, PLUS or Perkins).
- Direct Loan Exit Counseling - CIM is notified when this is completed.
- CIM Institutional Loan Promissory Note - Required if you have been awarded a CIM Institutional Loan and asked to complete by the CIM Financial Aid Office (international students only).
- Private Education Loan Applicant Self-Certification Form - Contact the CIM Financial Aid Office for the figures to report in Section 2 of this form prior to sending the form to the private lender you choose. For the 11-12 year, the Period of Enrollment Dates to be used for an annual loan are 8/29/11 to 5/10/12 (the first day of classes and the last day of finals).
- Verification Forms (only complete if asked to do so by CIM Financial Aid Office):
- Verification Policies sheet - details on the process of completing either the Dependent Student verification form or the Independent Student verification form.
- Dependent Students (generally this is for all undergraduate students; if not sure, check with Financial Aid Office).
- Independent Students (generally, graduate students).
- Verification Policies sheet - details on the process of completing either the Dependent Student verification form or the Independent Student verification form.
- Tuition Sponsorship Agreement Form - Used to help sponsors support a CIM student who has demonstrated financial need.
- FERPA Release Form - Required to permit CIM to speak to a parent, spouse, other person about student-specific information (such as tuition balance due, financial aid, grades, etc.) CIM must have the student consent in writing. FERPA regulations only apply to confirmed students (law does not apply to applicants until they have confirmed enrollment). All admitted students are sent this form, but need only return to CIM if/when confirming intent to enroll.

Financial Aid Office
Office hours are from 9:00 a.m. to 12 noon & 1:00 pm to 5:00 p.m. EST Monday through Friday.
NetPartner is the online Financial Aid system used at CIM. This site will be your main reference point once you are have formally applied to CIM and/or are a currently enrolled student: http://netpartner.cim.edu.
The CIM School Code for the FAFSA is 003031.
