From time to time, the Cleveland Institute of Music creates promotional and other materials that feature photographs, images, and videos of our students, faculty and staff. CIM uses these materials for fundraising, publicity and marketing purposes.

The materials created by CIM include, but are not limited to, the following: newsletters, mailings, articles, brochures, pamphlets, flyers, display photos, posters, videotapes, the CIM website, orientation training and programming materials, educational materials, promotional, marketing and fundraising materials, proposals, conference materials, displays, informational handouts, and advertisements. CIM may use these materials internally and externally. These materials may appear in or on publications, television, radio, trade shows, conferences, concerts, recitals, fundraising events, and other events.

By signing this authorization you are giving CIM permission to use photographs, images and videos of your child for the purposes described herein. You are also waiving any right to inspect or approve the finished materials and consenting to the possible broadcast or publication of these materials now or in the future. You agree that these photographs and or videos will become the property of CIM and will not be returned. You waive any right to any compensation related to the use of the photograph.

You may revoke this authorization at any time by notifying the Director of Marketing at the Cleveland Institute of Music in writing at the address listed below, except to the extent that CIM has already used or disclosed the photographs, images, or information in reliance on this authorization.

You release CIM from any claims related to this authorization.
